Submission of an intake form does not guarantee that a provider-patient relationship will be established or that a prescription will be issued.
All medical decisions, including whether treatment or a prescription is appropriate, are made solely by the licensed provider based on professional judgment and applicable law.
Telezen MD does not manufacture, compound, dispense, or ship medications. Prescriptions, if issued, are fulfilled by independent, licensed third-party 503A compounding pharmacies that operate in compliance with applicable U.S. laws and regulations.
Medications are dispensed and shipped by these partner pharmacies. Patients do not select the dispensing pharmacy, as pharmacy fulfillment is determined based on clinical appropriateness, availability, and operational factors. Pharmacy partners may change at any time based on regulatory, operational, or clinical considerations.
Telezen MD services are not health insurance and are not a substitute for health insurance. Telezen MD does not submit claims to Medicare, Medicaid, or any other federal, state, or private payor.
