NOTICE OF PRIVACY PRACTICES (HIPAA)

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

This Notice of Privacy Practices (“Notice”) describes how protected health information (“PHI”) about you may be used and disclosed and how you can obtain access to this information.

When this Notice refers to “Telezen MD,” “we,” “us,” or “our,” it refers to Telezen MD, LLC, a Florida limited liability company, and its role in facilitating telehealth services in coordination with licensed healthcare providers and third-party service partners.

Licensed healthcare providers who provide medical services to you through Telezen MD may maintain separate notices of privacy practices that govern their independent medical practices.

Our Legal Duties

We are required by law to:

  • Maintain the privacy and security of your PHI
  • Provide you with this Notice of our legal duties and privacy practices
  • Follow the terms of the Notice currently in effect
  • Notify affected individuals following a breach of unsecured PHI

We reserve the right to change this Notice at any time. If material changes are made, the updated Notice will be posted on our website and made available upon request.

How We May Use and Disclose Your PHI

We may use and disclose your PHI without your authorization for the following purposes, as permitted by law.

A. Treatment

We may use and disclose PHI to facilitate telehealth services, including sharing information with licensed healthcare providers involved in your care and, if applicable, with independent pharmacies for prescription fulfillment.

B. Payment

We may use and disclose PHI for billing, payment processing, and related administrative purposes.

C. Health Care Operations

We may use and disclose PHI for operational purposes such as quality assessment, compliance activities, audits, internal training, and administrative functions.

D. Treatment Communications

We may contact you regarding care coordination, treatment-related information, or health-related services that may be relevant to you, as permitted by law.

E. Individuals Involved in Your Care

Unless you object, we may disclose relevant PHI to family members, relatives, or individuals you identify as involved in your care or payment for care. If you are unable to agree or object, we may use professional judgment to determine whether disclosure is in your best interest.

Other Permitted or Required Disclosures

We may disclose PHI without your authorization in the following circumstances, as required or permitted by law:

  • To public health authorities for public health activities
  • For health oversight activities such as audits or investigations
  • In response to legal proceedings, subpoenas, or court orders
  • To law enforcement as permitted by law
  • To coroners, medical examiners, or funeral directors
  • For research purposes, subject to required approvals
  • To prevent or lessen a serious threat to health or safety
  • For military, national security, or correctional institution purposes
  • For workers’ compensation or similar programs

Uses and Disclosures Requiring Authorization

Any use or disclosure of your PHI not described in this Notice will be made only with your written authorization, which you may revoke at any time in writing.

Telezen MD does not sell your PHI and does not use PHI for fundraising purposes.

Your Rights Regarding Your PHI

You have the following rights, subject to certain legal limitations:

A. Right to Access

You may request access to or a copy of your PHI. Requests must be submitted in writing. Reasonable, cost-based fees may apply.

B. Right to Request Amendment

You may request correction of PHI you believe is inaccurate or incomplete. We may deny requests under certain circumstances, as permitted by law.

C. Right to an Accounting of Disclosures

You may request a list of certain disclosures of your PHI made during the six (6) years prior to your request.

D. Right to Request Restrictions

You may request restrictions on how your PHI is used or disclosed. We are not required to agree to all requests, except as required by law.

E. Right to Confidential Communications

You may request to receive communications about your PHI by alternative means or at alternative locations.

F. Right to a Paper Copy

You may request a paper copy of this Notice at any time.
